Enhanced Care Management & Community Supports

Enhanced Care Management (ECM) and Community Supports (CS) are specialized services designed to bridge the gap between healthcare and social needs for eligible Medi-Cal members. In collaboration with Health Plan of San Joaquin and Health Net, SJ Health is working to ensure that everyone has access and receives the quality care they deserve.

Our dedicated team of case managers support patients or their loved ones who are struggling to manage complex health conditions while facing housing instability.   We offer housing transition navigation, assist with housing deposits, and provide tenancy-sustaining services to ensure you stay housed. Beyond housing, our ECM services so you can focus on your health by connecting you with the appropriate health care providers, coordinating appointments, providing personalized care plans, and more.
