SJ Health accepts many health programs such as Medicare, Medi-Cal, and Family Pact.

SJ Health is part of both the Health Plan of San Joaquin and Health Net Medi-Cal provider networks in San Joaquin County. *We can help you select SJ Health as your provider if you are new to either health plan, or if you want to change your PCP.

No one will be denied access to services due to an inability to pay; and there is a discounted/sliding fee schedule available based on family size and income.

Sliding Fee Discount Program

What is it? Our Sliding Fee Discount Program is a payment program that allows eligible patients to pay for health services at a discount.
Do I qualify?
To see if you qualify for the Sliding Fee Discount Program we need information about you, including:
- o Your household income (before taxes).
- o Your family size.
How long will I be on the Sliding Fee Discount Program?
You can remain on the Sliding Fee Discount Program if you continue to qualify.
- o You will fill out a patient eligibility form at your first visit and again annually.
- o You must notify if there are changes to your income and/or family size.
How much will I pay?
Sliding Fee Discount Program fees differ depending on your income and family size.
